Electronic health records. If your doctor uses EHR software, it means you have an electronic health record containing your past and current medical history. Types of Balancing Valves

There are more than three kinds of balancing valves; however, these three are the most common. The calibrated manual balancing valve has a valve and pressure port on each side of the valve. Accretive employees, indistinguishable from hospital staff members, were allowed to help register patients and take down private health information, whereupon they would employ aggressive techniques to get patients to make an upfront payment. Documents released by the Minnesota attorney general’s office also show that Accretive collectors, calling themselves “financial counselors,” were instructed by management to stall patients trying to enter the emergency room until after they agreed to pay a prior balance. Most surprisingly, however, when overall costs to achieve a pregnancy were compared, highly touted “cost savings” with LI-IVF were nowhere to be observed. Lower pregnancy rates associated with LI-IVF meant that LI-IVF patients on average needed more cycles to have a take-home baby, which meant the lower per-cycle cost of LI-IVF did not translate into lower cost to achieve a successful pregnancy and delivery. Patients in LI-IVF cycles also took longer to conceive.

“LI-IVF appears to reduce pregnancy chances and prolong time to conception without offering any appreciable compensatory financial benefits,” summarizes Norbert Gleicher, MD, the study’s lead author and medical director of CHR. “When patients started asking for LI-IVF at our center, we, like many other IVF centers, decided to offer the procedure. In contrast to other centers, we, however, explained to our patients that LI-IVF should still be considered ‘experimental’ since no data existed, that compared this method of IVF to standard IVF.”

CHR required that patients sign “experimental” informed consents, and started accumulating outcome data.  Dr. Gleicher now notes: “As the result of our study well demonstrates, doing otherwise would have been rather unethical.”

“We must caution patients against resorting to ‘mini IVF’ or ‘natural IVF’ without carefully weighing their options,” warns David Barad, MD, another author of the study and director of clinical ART at CHR. “Until more data becomes available, physicians should offer LI-IVF only as an experimental procedure.”

Outstanding debt collections often result in selling unpaid bills to collection agencies, sometimes while the patient is still reviewing payment options.  If the amount owed is too small, a collection company might not continue efforts to track down payment, although they would still report the unpaid balance to credit agencies. According to research carried out by the Federal Reserve board, more than half of all collection accounts on credit reports are for medical bills, many of which are for less than $250. Most consumers are unaware that such a small sum can have a tremendous effect on their credit scores and can remain on their credit report for up to seven years.

A surprising find by the Commonwealth Fund, a private foundation that sponsors healthcare research, shows that of the 30 million Americans contacted by collection agencies in 2010, about half of them believed they were wrongfully contacted. In many cases, these patients were contacted for reasons outside of their own control, such as incorrect claims filing or improper billing notification on the behalf of the provider.

Conducted at the Center for Human Reproduction (CHR), a fertility center in New York City specializing in fertility treatments for older women, the study compared egg yields during in vitro fertilization (IVF) in women above age 40 with varying FMR1 genotypes and sub-genotypes.

In women with very poor ovarian reserve (i.e., women with the poorest ovarian function), the FMR1 sub-genotype het-norm/high (normal CGG repeat count on one allele, abnormally high count on the other) produced significantly more eggs than other genotypes and sub-genotypes. This observation suggests that the het-norm/high FMR1 sub-genotype preserves a woman’s ability to produce a good number of eggs at older ages even if the ovarian reserve is severely reduced.

“From our previous research, we knew that the het-norm/high sub-genotype was responsible for slow recruitment of eggs into maturation process at younger ages than other genotypes and sub-genotypes,” explains Norbert Gleicher, MD, lead author of the study and Medical Director of CHR. “Because these women ‘use up’ fewer eggs from their egg reserve, we suspected that they may have more eggs left when older. This study confirmed this hypothesis, demonstrating that women with this sub-genotype performed better in IVF cycles than even women with normal FMR1 genotype.”

These findings further enhance the understanding of genetic control over the process of ovarian aging, and further refine prognostication in older women undergoing fertility treatments. Given that oocyte yields in IVF cycles usually correlate with pregnancy chances, older women with extremely low ovarian reserve, therefore, appear to have better chances of success if their FMR1 sub-genotype is het-norm/high.

Researchers found  that the more meat you eat, the greater your risks are. The study, which was published this Monday in Internal Medicine, analyzed data sets from two studies conducted on over 121,343 men and women who completed health surveys between the years 1980 to 2006.  In the group, 23,926 people died, of which 5,910 from cardiovascular disease, and 9,464 from cancer.

Cardiologists in Florida at the Cardiovascular Institute of Northwest Florida, leaders in invasive and noninvasive cardiovascular treatment, help patients determine their ten year risk for having a heart attack.  Eating a lot of red meat has been on the list of foods to avoid for those at risk for heart disease. This study further demonstrates the need to reduce red meat consumption among these groups, especially processed meat like bacon, which was associated with an even great risk: 20 percent over all, 21 percent for cardiovascular disease and 16 percent for cancer.

The researchers could also include from the data analysis that if the people had eaten half as much meat, the researchers estimated that deaths in the group would have declined 9.3 percent in men and 7.6 percent for cancer.

Cardiovascular Institute of Northwest Florida cardiologists are among the top 10 percent ranking in the nation in treatment for heart attacks, coronary interventions and heart failure.

“Detecting DOR is not difficult,” says Dr. Gleicher, Medical Director and Chief Scientist of CHR. He continues: “Ovarian reserve can be easily measured by simple blood tests, like follicle-stimulating hormone (FSH) and anti-Müllerian hormone (AMH).  High FSH and/or low AMH suggest a diagnosis of DOR but both need to be assessed in an age-specific way.”

However, many fertility centers, unused to treating younger women with premature ovarian aging (POA; a term coined by CHR to describe younger women with DOR), often overlook this condition. “They overlook the diagnosis,” explains Dr. Gleicher, “because they do not utilize age-specific cut off values for FSH and AMH.”

Normal FSH ranges increase and normal AMH ranges decrease as women age.  If all women of all ages are assessed with universal cutoff values, younger women will never be diagnosed in timely fashion. “Unfortunately,” says Dr. Gleicher, “this is what is still done in many fertility centers.”

Once diagnosed with POA, like older women, women with POA have little time to lose since there is no telling how quickly their POA will progress. Though very rapid progression is rare, according to Dr. Gleicher, CHR physicians have seen women go into full menopause in a few short months after diagnosis.

Dr. Gleicher continues: “There isn’t a day when we don’t hear our patients say ‘doctor, I wish I’d known about your center months ago, when I was doing such and such…’ The earlier we can start treatment, the better, of course, our chances of helping our patients with POA!”

CHR’s tips for younger women with POA to receive timely diagnosis and treatment include the following:

• Be very skeptical of the pseudo-diagnosis of “unexplained infertility.” Especially in younger women, this diagnosis only means that a diagnosis of POA was missed.
• Insist on age-specific ovarian reserve testing with FSH and AMH.
• If your age-specific FSH and/or AMH values are abnormal before age 40, you likely suffer from POA.
• If that is not clearly spelled out to you, your physician may have little experience with diagnosis and treatment of POA, and it may be time for a second opinion.
• Remember that with a diagnosis of POA, time is crucial, because nobody can predict how quickly your ovarian reserve will further decline.

According to the article on Government Health IT, physicians reported to RECs that the biggest obstacle they face in meeting EHR meaningful use criteria is waiting for vendors to upgrade their software or get ONC-ATCB certified.  The vendor selection dilemma accounted for 34 percent of all reported barriers.  The problem does not appear that there aren’t any vendors who meet meaningful use, but quite the opposite.  ONC staff member Dawn Heisey-Grove believes there to be too many vendors to choose from to the point that it works to the physician’s detriment.  “My general sense is I think it may be an overabundance of choices.  Hundreds are certified.  And a large portion of providers were doing rip-and-replace,” Heisey-Grove told the meaningful use panel of the advisory Health IT Policy Committee last week.

Despite the reported roadblocks, 26 percent of reporting physicians said they are still on track to attesting meaningful use.  The data from the REC reports was collected to examine how physicians are handling EHR implementation and Stage 1 meaningful use.  These meaningful use attestation barrier reports may be used as empirical evidence to support testimonies in public hearings.   ”One of the important things for meaningful use is understanding real world experiences, and this is some of the first real-world data we’ve gotten and provided in a quantitative way,” said Josh Seidman, director of the ONC’s meaningful use division.

“It’s important to recognize the urgency of aggressive fertility treatment when you are above age 40,” says Dr. Gleicher, Medical Director of CHR, which released the tip sheet. • Recognize the urgency of aggressive fertility treatment when you are above age 40.
• Insist on rapid diagnosis and a structured treatment plan.
• Insist on specific treatment goals that meet your expectations.  For example, do not agree to treatment with clomiphene citrate and intrauterine insemination (IUI) if the expected pregnancy chance sounds ridiculously low.
• Do not agree to endless testing to “wait for the right results” – time is not on your side, and results will only get worse!
• It never hurts to get a second opinion.

First step when measuring the balance is to make sure all the thermostat controlled vales on the radiators are open completely and that your system is at its normal working temperature. It is important to make note that a properly balanced system does not have all the radiators at the same temperature. The temperature of the radiator depends on the size of the room it is heating, the air flow of the room and if doors are opened, its set temperature and the size of the radiator. Additionally, make sure you take into account that in general the temperature of the radiator is a bit higher at the top than at the bottom.

As first reported by Dr. Gleicher’s team in 2006^(1,2), European IVF pregnancy rates are significantly lower than American rates.  The most recent European data show clinical pregnancy rate per cycle of 32.5%. Conservatively assuming a miscarriage rate of 15%, this would translate into delivery rates of 26.6%. During the same time period, the US delivery rate was 40.7%.

The question is why.

“A part of the reason is the recent propagation of single-embryo transfer (sET) in Europe,” explains Dr. Gleicher. “In Europe, fertility experts and governments alike consider the reduced risk of multiple pregnancies through sET a good enough reason to offset lower pregnancy chances caused by sET. Thus, many European countries mandate sET despite irrefutable evidence that patients value nothing more than better chances of pregnancy in choosing their infertility treatments.

“This is a good example for the fundamental difference in the socio-medical philosophy between Europe and the US,” points out Dr. Gleicher: “Europe values what they perceive as the ‘common good’ over individual rights of patients. What they fail to recognize, however, are the unintended consequences affecting the patient-physician relationship and, most importantly, IVF outcomes.”

He continues: “Not only do patients end up with much lower pregnancy chances with IVF than their U.S. counterparts, they, in the end, are also forced to undergo more IVF cycles , at more cost, and, of course, more risks.” Europe, therefore, performs more than three-times as many IVF cycles per capita than the US. The same philosophy in Europe also results in outright rationing. For example, most Scandinavian countries withhold all IVF treatment from women above age 40-41, an approach unthinkable in the US.

“If there is a lesson to be learned here,” says Dr. Gleicher, “it is that IVF pregnancy rates in the U.S. will continue to improve, as long as IVF research and clinical care remain largely unaffected by government interference, and primarily driven by success in the marketplace, as they have over the last few decades.”

The scientists in this study isolated, purified, and treated human colon cancers cells and found that as they subjected the cells to the derivatives from mate tea, caffeoylquinic acid (CQA) at higher concentration, the cells died through apoptosis, the process of morphology and programmed cell death due to biochemical events.  In this process the cancer cells’ DNA becomes damaged

The survey entitled “Patient Interest in Sharing Personal Health Record Information,” found that the majority of patients are interested in knowing what physicians and relatives have to say about their health conditions. Specifically it found that 79 percent of the 18,000 users of My HealtheVet want to share the information in their patient health record with someone outside of the VA system.

Of the 79 percent wanting to share their information, 62 percent wanted to grant access to their spouse or partner, 23 percent to one of their children, 15 percent to another family member and 7 percent to an unrelated caregiver.

Norbert Gleicher, MD and Medical Director of New York City’s Center for Human Reproduction (CHR) – a leading clinical and research center in infertility – has recently brought attention to the rising number of older women becoming mothers. In a blog published by CHR he notes that a number of media reports recently presented the pros and cons of pregnancy in older women. All reports, however, missed the most important conclusion to this seemingly sudden societal development (which in reality has been growing quietly for over a decade): the developed world is in the midst of a reproductive social revolution in which we will increasingly see older, and often single, women becoming mothers.

So far, the medical profession, academia and government have failed to address potential societal consequences of an increase in older mothers. The public and medical establishments are similarly skeptical and to a degree hostile to what some have derisively called “grandmothers having children.” Yet, Dr. Gleicher notes, “The trend [of pregnancy in older women] is irreversible, and can only be expected to accelerate.”

Dr. Gleicher further points out that at CHR the median patient age, which a decade ago was around 35 years, passed 40 in 2011. Egg donation, mostly utilized by older women who no longer have use of their own eggs, is CHR’s most rapidly growing in vitro fertilization (IVF) program. Trends also can be seen nationally based on Center for Disease Control and Prevention data. Between 2004 and 2008 percentages of IVF cycles as a proportion of all IVF more than doubled above the age of 42. By 2008, egg donation cycles already represented 12.3% of all IVF cycles in the US.

“Medicine is not ready to manage pregnancy in older women safely and society is not ready to help them cope with older motherhood,” warns Dr. Gleicher. “Affected medical specialties have to develop the necessary expertise, whether they agree with patients’ decisions to be pregnant at advanced ages or not.” Feeling strongly about the subject, he concludes, “As we do not withhold care from smokers with lung cancer or from overly obese diabetics, it would be unethical to withhold care from older women desirous of motherhood.”

Advantages and Risks of a Facelift

In a paper just published in the journal Reproductive Biology and Endocrinology¹, researchers from the Center for Human Reproduction (CHR) reached these conclusions combining results from human and recent mouse research. Some androgens, such as testosterone and dehydroepiandrosterone (DHEA), may make essential contributions to the normal maturation of follicles that contain eggs in the ovary.

Androgen’s role in female fertility has been controversial at best. Some researchers have even insisted that androgens are the principal enemy of follicle maturation, especially given observations in patients with polycystic ovarian syndrome (PCOS), who often present with excess androgens and tend to struggle with infertility.

“Recent research data suggests that androgens and FSH, likely at specific, balanced concentrations, work together in promoting follicle development,” explains Norbert Gleicher, MD, lead author of the paper, and Medical Director of CHR. “Investigations of such intricacies of ovarian function cannot be done in humans, at least initially, and requires integration of animal data, which is exactly what we did.”

The synergism between androgens and FSH appears most pronounced in the pre-antral and early antral stages of follicle maturation, at least 2-3 months before an egg is ready for ovulation. “This may open new avenues for treatment, especially for women with ‘older’ ovaries,” says David H Barad, MD, another author of the study. “Instead of focusing only on the last two weeks of oocyte maturation, as we currently do with infertility treatments, we may be able to offer treatments that benefit follicle development from much earlier maturation stages.”

Dr. Gleicher notes in the publication that doctors at CHR have preliminary evidence that longer-term exposure to DHEA in combination with FSH may positively affect oocyte development, though more definite conclusions require further studies.

3. Check to see whether you have two automatic balancing valves coupled together very closely.  If two of these kinds of valves are close-coupled together, they can create pulsing and noise.  The only way to solve this issue would be to uncouple them and rework the system.

As an infertility expert who specializes in “older women” he was particularly touched by the hopelessness that Ms. Finn conveyed in her essay.

“As a gynecologist and infertility expert, I – like most of my colleagues, of course – constantly try to create awareness about the importance of female age for successful reproduction,” he said. “The younger the better! But, increasingly, every time I make this point to an ‘older’ patient, I am uncomfortable about unwittingly aggravating her already unwarranted and excessive sense of guilt that is so well depicted in Ms. Finn’s essay.”

According to Dr. Gleicher there is no clear answer to how many infertility treatments are too many. He says that this depends wholly upon the individual circumstances of each woman. He does say, that this woman’s chances could be much greater should she choose to go with a donated egg.

“While genetic inheritance very clearly represents one of the primary purposes of reproduction, biological maternity is also of crucial importance to maternal-child bonding,” explained Gleicher. “Women today are no longer limited by their own reproductive lifespan (i.e. their ovaries’ ability to produce viable eggs) because (anonymous) egg donation has become widely available. In the United States, egg donation now represents the most rapidly growing fertility treatment within IVF.”

A majority of failed IVF cases (and miscarriages) are considered to be a result of chromosomal abnormalities in embryos. In a smaller number of cases—estimated by some authorities to represent approximately 15% of failed IVF cycles—implantation fails because the process of implantation itself malfunctions. “Implantation is an immunologic process,” explains Norbert Gleicher, MD, Medical Director at the IVF NY center CHR and an international expert in the immunology of reproduction. “Because the genetic material of the implanting embryo is half paternal, for the mother’s immune system, the embryo is analogous to an organ transplant. Under normal circumstances, however, this ‘transplant’ is not rejected.”

Why the maternal immune system tolerates this “transplant” is also not well understood. However, recent research at CHR suggests a genetic component. Investigators at CHR recently closely linked a gene on the X-chromosome, the so-called FMR1 gene, with ovarian aging and chances of conceiving with IVF:  women with so-called normal genotype had almost double the pregnancy chance with IVF as women with so-called heterozygous-normal/low genotype.¹

A follow-up study, also in the medical journal PLoS ONE, suggested that FMR1 differences are also likely responsible for variations in IVF pregnancy rates in different races/ethnicities.²

David H. Barad, MD, the Center’s Clinical Director of IVF explains: “We now have evidence that IVF outcome differences associated with FMR1 affect implantation. They are likely immunologic in nature.” These are the first reported genetic controls for IVF outcomes. “This genetic component to embryo implantation may lead to prognostications, based on IVF patients’ genetic profiles,” adds Dr. Gleicher. “Potentially, then, these prognostications may open up revolutionary new avenues to therapies to improve implantation and IVF pregnancy chances through immunologic treatments.”

What is Prolotherapy?

Premature Ovarian Failure (POF) is also called early menopause or Primary Ovarian Insufficiency (POI), a recently coined medical term. Current diagnostic criteria for POF are Follicle Stimulating Hormone (FSH) levels above 40.0 mIU/mL before age 40 – a condition known as High FSH. True POF affects only approximately 1% of all women. However, approximately 10% of all women experience a premature decline of ovarian function, which is milder than outright POF. This milder decline in ovarian function is called Premature Ovarian Aging (POA) or Occult Primary Ovarian Insufficiency (OPOI).

This multitude of medical terminologies for similar or identical medical conditions has led to serious problems for affected patients. “Correct diagnosis is crucial in advising patients about prognosis and treatment choices, because women with POA, especially if under age 42, still have excellent pregnancy chances with correct treatments,” explains Norbert Gleicher, MD, Founder and Medical Director of the Center for Human Reproduction (CHR) in New York City. “In contrast, women with true POF have much poorer pregnancy chances with the use of their own eggs, and usually require experimental treatments.”

Botox and Dysport have more similarities than differences.  Both medications are made from the protein produced by the bacteria Clostridium botulinum.  This protein is a neuromuscular blocking toxin that relaxes targeted muscle areas.  They are injectable solutions applied to the face and other areas to reduce the appearance of skin wrinkles caused by aging.

Barad, MD, Clinical Director of IVF at CHR. “If the number of eggs, instead of ovarian reserve, were the true determinant of live birth chances, we could simply stop retrieving eggs when we reach that specified number. This, however, would only increase risks to patients. Perfect oocyte numbers, therefore, cannot really be acted upon.”

Dr. Gleicher also warns that patients should not be misled into believing that every patient undergoing IVF can or should be stimulated toward the production of 15 to 20 eggs. Very few women above age 40, for example, can ever reach this range, even with maximal ovarian stimulation. Young women with polycystic ovary syndrome (PCOS), on the other hand, will often exceed this range, even with the mildest stimulation. “It is not the number of eggs we worry about,” says Dr. Gleicher. “It’s the patient’s ovarian reserve!”

Dr. Damian Dowling from Monash University in Melbourne and his colleagues conducted genetic studies on fruit flies to determine how genetics affect male infertility. Fruit flies have a surprisingly similar genome to humans, making them an appropriate subject of study.

In looking at the power-center for cells, known as the mitochondria, the researchers realized that the genetics that make up this part of the cell don’t go through rigorous quality control checks like other genes do. The result is high levels of genetic mutations that go unchecked and get passed along to male offspring.

“What our results suggest is that the mitochondria have inadvertently evolved to be bad for males, but good for females, as a by-product of their maternal transmission,” said Dowling. “While medical practitioners have a fairly good idea that certain mitochondrial mutations can bring about male infertility, the evolutionary process that we uncover actually suggests that the mitochondria might well harbor very many unidentified mutations, all of which could contribute to the problem of male infertility.”

Although this study does not necessarily explain all male infertility, it is a breakthrough in how researchers may study this condition going forward. Dowling concludes, “At the very least, our results should therefore provide a road-map of where we should be looking for these mutations.”

Male infertility can be related to issues of low semen count, low semen volume, or abnormal sperm. This research study was published in the May 13, 2011 edition of the journal Science.

They state that the statement is for clarification purposes, as there are many public misconceptions about compensation for egg donations.

It has long been known that IVF success rates differ amongst different races/ethnic groups. This new study suggests that predisposition to autoimmune disease may be the cause for these differences.

Despite general improvement in outcomes of fertility treatments, disparities between races/ethnicities have actually increased. It is most important to balance the intake of omega-3 and -9 fatty acids (found in finfish, shellfish, chia seeds, flaxseed oil, dark leafy greens, and certain nuts, olives and avocado) and the intake of omega-6 fatty acids (found in refined vegetable oils, many processed foods, and meat).  The American diet tends to be deficient in omega-3’s and overloaded in omega-6 fatty acids, causing a number of chronic health conditions – including heart disease.  A good diet for heart disease prevention and treatment would attempt to rebalance omega-3 to 6 ratio.

Omega-6 fatty acids are also known to have pro-inflammatory properties, making them an equally poor dietary choice for a chronic pain patient.  Increasing the intake of omega-3’s, which have anti-inflammatory properties and can help supplement a more aggressive pain treatment program. G-CSF is a cytokine (protein molecules that facilitate communication between cells) that has already been FDA-approved for other clinical indications.

Researchers from New York City-based Center for Human Reproduction (CHR) and a second NY fertility center reported on four women with highly inadequate endometrium, unresponsive to conventional treatments and facing IVF cycle cancellation. In all four cases, endometrial perfusion of G-CSF expanded the endometrium to a minimal thickness of 7mm within approximately 48 hours, allowing for embryo transfer.  Surprisingly, all four patients also conceived.

In approximately 1% of all IVF cycles, the endometrium stays too thin for embryos to successfully implant, even with treatment. In such IVF cycles, patients and physicians face two poor options: they either accept lower pregnancy chances and transfer embryos despite inadequate endometrium, or they can cancel the embryo transfer and freeze all embryos in hope of better future cycles. Unfortunately, women with inadequate endometrium often show inadequate endometrium again in future cycles. This situation was demonstrated by two patients in this reported case series with histories of repeat IVF failures due to thin endometrium.

“Conventional treatments for inadequate endometrium have had spotty success at best. Without G-CSF perfusion, these patients, likely, would not have reached embryo transfer,” explains David Barad, MD,  Director of Clinical ART at CHR and one of the senior authors of the report. “That all of them also conceived was a big surprise, and is, of course, quite remarkable.”

“The efficacy of G-CSF in improving endometrium, and possibly pregnancy rates in general, still needs to be confirmed in randomized controlled trials,” adds Norbert Gleicher, MD, lead author of the report and Medical Director of CHR. “Indeed, we already started two such trials to test both hypotheses; but until first results become available later in 2011, we caution against over-interpreting results of this small pilot study.”
